What
is a Life Care Manager?
A
Life Care Manager is a certified or licensed
human services professional who acts as a guide for
older adults, families and caregivers caring for older or disabled
persons. We provide continuity of
care by coordinating communication between family,
health care and other providers. We design an
individualized plan of care based on the client's
needs and monitor the care.
We
listen to older adults and their families to assess
and monitor needs, solve problems, plan for future
needs, and coordinate in-home services. Special
Care has a team of dedicated
professionals who understand the complexities of
aging, health care issues, Alzheimer's and related
dementias. Our services also extend to veterans, persons with mental illness, developmental and
other disabilities.
In
partnership with your family we:
- Coordinate
care and advocate for your loved one
- Control
quality of care based on the national standards of
practice and code of ethics
- Contain
costs by avoiding inappropriate placements,
duplication of services, and unnecessary hospital
stays
- Respect
the desire to "age in place" while
remaining safe and secure
Specifically
we:
- Manage
medical care and medications
- Evaluate
and help choose housing options
- Identify
providers and monitor personal care services
including grooming, bathing and dressing
- Connect
with elder care assistance programs
- Make
referrals to elder law attorneys, health care
providers and other professionals
- Coordinate
assistance with meals, nutrition and weight
management
We
help with Household Management:
- Identify
providers and monitoring quality of home services
including laundry and housekeeping
- Assist
with errands, phone use, and moving
- Identify
transportation options for appointments
- Monitor
delivery of home maintenance services (i.e.
cleaning, laundry, lawn
care and snow removal)
- Set
up systems to have mail sorted and bills paid

- Social
and other activities customized to the person's
interests
- Make
telephone checkups
- Help
write letters, read and answer email
- Coordinate
simple holiday and religious observances
